Form Instructions
Use this form to estimate costs or use it as the formal order form. There is NO hidden charge. This order form is still in the trial period, Canaan Media reserves the right to correct any errors caused by it. If you have any questions call us at (908)218-7929 If you don't know the exact length of a tape, pay us only the minimum charge now. We'll bill you late if the tape is longer than what you paid. Send this form to us, also print 2 copies. Keep one and send the other one with your order. Number your audio tapes from 1 to X and we'll follow that order. Ship us your tapes 2-3 day air w/ tracking (UPS, FedEx, USPS Priority) Our average turnaround is 14 days, RUSH 7 days. We will ship your new media & tapes 2-3 day air with tracking. We keep digital backup for 30 days after shipment. Mail tapes, order form and payment to the address above.
